The Township of Teaneck requires all players, coaches and officials using Township fields to complete a health questionnaire including a temperature screening before entry into sports facilities and fields.
Health Screen Questionnaire
Visiting players, coaches, and officials must print and bring a completed Health Screen Questionnaire form to the Teaneck field
Completed form will be collected by a Teaneck representative at the field's entry gate
Parent/guardian will be required to complete the form for children under 18 years of age
Participant will not be permitted entry to the facility or field if the questionnaire is not completed or if any health risk factors are identified in the questionnaire responses
Temperature Check
Each participant will be subjected to a contactless Temperature Check before the participant enters a field or facility. If the participant’s temperature is 100.4F or higher, s/he will not be permitted to participate in soccer activities.
